Scheduling, Payment, & Insurance
- Physical therapy services are currently out of network with all insurance plans. If requested, we will provide you with a superbill and you may submit this to your insurance company for reimbursement.
- Direct patient care gives us the ability to tailor our services for your needs rather than limiting and restricting treatment needs as often dictated by insurance companies. This allows more quality time during your therapy sessions as there is less documentation required, as well as transparency with pricing.
- Online booking allows you the freedom to set up your appointments when it's convenient for you. A deposit is required to schedule your appointment. This deposit is transferable if your appointment is rescheduled up to 48 hours before your appointment time. Payment in full is processed on the day of service to the card on file.
- We do not meet the criteria for reimbursement by Medicare or Medicare replacement plans. You may still elect to have care provided by Purely Pelvic LLC.
